- Use the following link to fill out your entry form on-line.
- Our database will be automatically updated with your information. After the information is submitted, you will receive confirmation vial email that it was received.
OPTION B: PRINTABLE ENTRY FORM
- Open the printable form below.
- Fill it out electronically and save file OR print the form and fill it out manually.
Step 2: Complete Hold Harmless, Payment Agreement
- Open the printable form below.
- Fill it out electronically and save file OR print the form and fill it out manually.
- REMEMBER TO SIGN THIS FORM.
Step 3: SEND in the Completed Forms and Payment
- Send the following to the postal address listed below.
X - Complete Payment of Entry Fee
X - Entry Form from Step 1 (Only required if not completed on-line.)
X - Hold Harmless, Payment Agreement from Step 2
X - Copy of Drivers' License and Insurance (Only required if you are taking a vehicle through the parade.
